Change of Supplier Name or Tax
Identification Number (TIN)
Change of Supplier Name must be submitted to update the supplier record
with the new name in OAKS FIN.
Change of Tax Identification Number is generally done when businesses
need to change the Tax Identification Number (TIN) when there has been
a change in ownership or structure (due to the purchase of a business,
a merger, acquisition, etc.). The Supplier Operations team will use the
completed form to obsolete the supplier record with the previous TIN in
OAKS FIN and create a new supplier record with the new TIN.


Review Form
Verify the "Change of Supplier Name or Tax Identification Number
(TIN)" is complete.
- Section 1 - New
Supplier Name and Tax Identification must be completed and
must match the Supplier Information Form (OBM-5657)
and Federal
W-9. The DBA name also must be listed if applicable. The supplier
name will be updated on the Identifying
Information tab in the Supplier Module.
- Section 2 - New Remit To Address
- Section 3 - Previous Supplier
Name and Tax Identification must be completed and must
match the name listed on the Identifying
Information tab in the Supplier Module. The DBA name
also must be listed and match the name listed on the Identifying Information tab
in the Supplier Module.
- Section 4 - Status of Previous
Tax Identification Number must be completed.
- If "Yes" is selected, an explanation must be provided
in Section 3.
- Section 5 - Reason For the Tax
Identification Number Change and Additional Comments must be
completed.
- Comment must indicate that they are providing different services
if they are keeping
their previous Tax ID active.
