OBM Financial Support Services Payment Request Form

The Financial Support Service (FSS) Payment Request Form (PRF) is used for regularly scheduled payments (typically grants, subsidies, rent payments, etc.) and stands in the place of the invoice for agency obligations. If the invoice is a FSS PRF, the Last Receipt Date is determined based on the Payment Period dates listed on the FSS PRF.
